plan to send the test results to the Premier Monitor annually, please
fill in the registered name of each dog you have in your breeding program below and
include copies of hip, eye, heart and patella test results for
each. For those who plan to certify all dogs with OFA/CERF,
only include the test results of those dogs who are not presently listed
in the databases and attach copies of their test results as well--this
includes all PennHIP results since they do not have a database available
to the public.
This is only for dogs you OWN, not those you plan to breed to who are
owned by others. Please double check to be sure clearances are
up-to-date before sending. |
